Transcript request for students that graduated from FCSD prior to and including 2017:

If you would like to request a HS Transcript or Immunizations please complete and submit the HS Transcript Request Form - only use this form if you graduated prior to and including 2017.

*Official transcripts are stamped, sealed and sent to colleges or employers.

*Unofficial transcripts are for your records only.
